Grant Testimonial Form

For good and valuable consideration, the receipt of which is hereby acknowledged, I hereby authorize the Louisiana Hospitality Foundation permission to use my name and/or likeness in a photograph and/or video (“media”) in any and all of their publications, including but not limited to all printed and digital publications, websites and social media. I understand and agree that any media using my name and/or likeness will become property of the Louisiana Hospitality Foundation and will not be returned.

I acknowledge that since my participation with the Hospitality Worker Crisis Grant Fund Program is voluntary, I will receive no additional financial compensation in exchange for use of my name and/or likeness.

I hereby irrevocably authorize the Louisiana Hospitality Foundation to edit, alter, exhibit, publish or distribute this media for purposes of publicizing their programs or for any other related, lawful purposes. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the media.

I hereby hold harmless and release and forever discharge the Louisiana Hospitality Foundation from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I am at least 18 years of age and am competent in my own name. I have read this release before providing consent and I fully understand the concerns, meaning, and impact of this release.
